The Perak State Health Department (JKN) has launched an investigation into the tragic death of a three-year-old girl who reportedly passed away after receiving multiple injections at a hospital in Teluk Intan last week.

Perak’s Exco for Human Resources, Health, Indian Community Affairs, and National Integration, A. Sivanesan, confirmed that the investigation is still in its early stages. He assured that there is full transparency in the process and that appropriate action will be taken if any negligence is found.

. A preliminary review was conducted on the day of the incident, and the child’s body has since been cremated,” he said.

Sivanesan emphasized that there is no attempt to conceal any details, expressing deep sympathy for the grieving parents. “We understand the immense loss they are facing. The JKN director and I are truly saddened and extend our condolences to the family,” he added.

Speaking at a public health briefing, Sivanesan revealed that the child's father had reached out to him on Sunday, informing him that the incident had been widely shared on social media by a family member. He advised the father to file an official complaint if he believed there was any wrongdoing in the medical treatment provided.

He also reassured the family that there would be no cover-up, as the Ministry of Health (MOH) could assign an independent medical professional to investigate the case if necessary.

As of Sunday, no official police report had been lodged by the child's family. The authorities urge those affected to come forward with any concerns so that the matter can be thoroughly examined.
